A person-centred record that reads like a story, not a form. Twenty-four care plan sections with pictures for easy-read, important people, documents in folders, visits, goals, a memory book, and a one-tap profile PDF that inspectors and families both understand.

Personal information, about me, communication, capacity and consent, medical history, medication, physical health, personal care, nutrition, mobility, sleep, mental health, positive behaviour support, risk, activities, independence, relationships, safeguarding, community, emergency, end of life and your own custom sections. Completion tracking and who-updated-when on every one.
Digital care planning, sometimes called electronic care planning, means the plan lives in one system instead of a binder. In Kiwi, each section can have its own review date and shows when that review is overdue, and earlier versions are kept when it changes. A member of staff can sign the section, and Kiwi shows who signed last and flags it for re-signing if it changes.

Every plan can be downloaded two ways: the standard black-and-white corporate document for professionals and printing, and an illustrated easy-read version made for the person, their family and advocates. Same content, two audiences.
Hospital passports, consent, DoLS, letters, PBS plans, nutrition charts. Blue folder tiles six levels deep, drag files from the desktop, take a photo on the phone, one tap creates the standard folder set. Nothing is deleted, only binned and restorable.

Family, advocates, GP, social worker and every professional who visits. A visit by a family member writes itself onto the daily log. The profile PDF lets you tick the sections you need and produces a corporate A to Z document in seconds.

When someone goes into hospital, one tap prints a transfer summary from the records you already keep. Page one is always a single page: resuscitation status, allergies, who to call, why they are going and their latest observations. Then their medicines with the last dose given, the MAR chart, and what matters to them. Anything that has not been recorded is printed as not recorded, so nobody has to guess.

The easy-read HAP and PCP documents are made for exactly that. Families and advocates get an illustrated version in plain language.
Yes. Free set-up: we send you a list of what we can import for you, then load it.
Permissions are per person, per module, read or write. Staff assigned to a home see that home only.
Software that holds each resident's care plan electronically instead of on paper. Sections can be reviewed and updated as needs change, earlier versions are kept, and staff can read the current plan on a phone or tablet. In Kiwi the plan has 24 sections, including About me, capacity and consent, positive behaviour support and end-of-life planning.
Yes. Both terms mean keeping care plans in software rather than on paper. What matters when you choose is whether the plan is easy to keep current, shows when reviews are due and keeps a history of changes for inspection.
Kiwi's eMAR was built by a former registered manager who previously ran learning-disability, mental-health and nursing homes, and supported-living services, during his career.
