Changes to care records such as daily logs, incidents and care plan sections now keep the earlier version, with who changed it and when: look for "Edited · History" on the record. A permanent audit log shows managers who signed in, opened, downloaded or changed what, and it is kept for eight years. The new Hospital Passport prints a hospital-ready transfer summary from the records, in a standard or an easy-read version. 1:1 and 2:1 hours now show who recorded them, and handovers belong to a home.
